Insurance & Payment Policy

Payment, including deductible or co-pay, is due at the time of service.  We are contracted providers for many insurance companies and will submit claims on your behalf, however, fees that are not covered by your insurance company must be paid in full.

The following information is required prior to the start of services: 

• Physician referral for occupational therapy and/or speech therapy

• Prior authorization from the insurance company

Cancellation Policy

Please notify us 24 hours in advance if you need to cancel your appointment.  There will be no charge for sessions canceled at least 24 hours in advance.  For sessions canceled with less than 24-hour notice, a cancellation fee may be charged. Failure to attend three consecutive sessions may result in termination of services.  Missed appointments and late cancellations are costly for us and greatly affect the outcome of therapy.

Weather Policy

The clinic will be open for scheduled appointments except in cases of severe weather. In case of bad weather please call our office to see if we are open or to let us know if you believe conditions prohibit you from driving safely.  Families may cancel treatment if they do not wish to travel because of poor road conditions.
